Nomination Form
To qualify as a potential recipient of our donations all following criteria must be met:

The organization must be a 501.c.3, OR a specific program within a 501.c.3 organization that serves as its fiscal sponsor.

If the nominated program is under an umbrella 501.c.3 organization, 100% of our donations must go to winning program or project,

If the umbrella nonprofit is faith or interfaith-based, *no recipient is to be exposed to or required to participate in religious activities, doctrines, or beliefs, and * checks may include the program name on the “Pay to the order of” line.

The nominating member must be present or may name 1-2 other members to give the presentation and lead the Q&A.
